You'll need to bring the following to the parent meeting:

Sportsmanship form - each player and parents sign and coaches keep.

Release of liability form – turn into registrar who will keep this form.

Birth certificate - coaches should have a copy. May be checked to verify player's age at a tournament.

Participants in Norfolk Express Soccer pay $65 per child. This includes $30 which is a tax-deductible donation.Board members will pay $45 per child playing. Head coaches pay $45 for the child they are coaching.

This also includes the fee that is paid to state and the insurance coverage. Each team will pay for the tournaments they want to go to by dividing the tournament registration fees amongst the players who are not head coaches' or board members' children. Teams are required to participate in the Norfolk tournament.

Twenty dollars will be charged to each family but will be refunded if the family participates in the fundraiser selling a minimum of four cases of Pepsi products. We will have cash prizes for those who sell the most cases.

Each team will still have a $50 equipment allowance, except for new teams, who get a $100 allowance.

Medical release/registration form - Registrations should be filled out and fees paid by March 1, 2010 .

Although it means writing multiple checks, parents, please pay for each child playingindividually, so that each coach can turn in their money and registration forms as a team.

Coaches turn in the registration forms and the money for all of the players to the registrar. The registrar will not submit your team information to state until all players are paid. The registrar will get the official stateroster and player passes and get them to the coach. Coaches put the picture on and laminate passes.

The player passes and a copy of the state roster must be presented at each tournament.

This form will be returned to the coaches as it is the medical release which also must be presented at tournaments.
